Symptoms

Hip pain

Lateral and posterior hip pain frequently of muscular origin — often labeled 'bursitis' without clear evidence.

Overview

Lateral hip pain is commonly produced by gluteus medius trigger points and tendinopathy. Posterior hip pain frequently involves the piriformis. Many cases diagnosed as 'trochanteric bursitis' on clinical grounds alone are primarily myofascial.

Clinical notes

Single-leg stance for Trendelenburg, side-lying abduction testing, palpation of greater trochanter and gluteus medius belly.

Red flags — when to refer

Acute trauma with inability to bear weight, suspected fracture, fever, progressive neurological signs — refer.
