Does dry needling work? What the meta-analyses show

Pooled evidence supports short-term pain reduction versus sham or no treatment, with smaller and less certain effects at longer follow-up.

The consistent finding

Across several systematic reviews and meta-analyses, dry needling of trigger points reduces pain more than sham or no treatment immediately and in the first weeks, and improves pressure pain threshold. Effects versus other active treatments are smaller and often not statistically distinguishable.

Estimates vary with the body region, the comparator and the quality of blinding, which is why individual trials disagree more than the pooled picture does.

Where the uncertainty is

Long-term outcomes are the weak spot. Many trials follow patients for days or a few weeks, sham needling is hard to blind convincingly, and sample sizes are small. Reviews repeatedly rate the certainty of evidence as low to moderate.

The reasonable reading is that needling is a useful short-term tool inside a broader plan, not a stand-alone cure.

How to use it in practice

Use needling to open a window of reduced pain and improved range, then spend that window on load management, specific exercise and correcting perpetuating factors. Re-evaluate after two or three sessions instead of committing to a long course.
