How reliable is trigger point palpation, really?

Systematic reviews find tenderness and pain recognition reasonably reproducible, taut band and twitch response much less so — which changes how you should examine.

The reviews and what they measured

Two systematic reviews examined whether different examiners agree when identifying trigger points. Both concluded that no single physical examination item has been shown to be reliable enough to serve as a stand-alone diagnostic criterion, and that study quality was often poor.

Agreement was highest for tenderness at a specific spot and for the patient recognising the pain, and lowest for palpating a taut band and eliciting a local twitch response.

Part of the problem is definitions

A review of diagnostic criteria found striking variability in what studies actually required before calling something a trigger point — different combinations, different numbers of criteria, sometimes none stated. Poor agreement between examiners partly reflects examiners being asked different questions.

This is why consistent templates matter: every muscle page here uses the same palpation description and the same confirmation step.

Practical consequences

Weight your examination toward the items that reproduce reliably: find the tender spot, ask whether the pain is the patient's familiar pain, and check whether the reproduced area matches the muscle's referred pain map. Treat the twitch response as a useful bonus, not a requirement.

Then judge the hypothesis by the response to a treatment trial rather than by how confidently the nodule was felt.
